Some First Star Capital funding programs may require the completion and submission of one or more of the forms below. If requested by your First Star Capital representative, please complete the appropriate form below. Completed forms can be submitted to our office via fax or e-mail depending on what is more convenient for you.

This document is the starting point for all new financing requests. Applications can be submitted either online or offline. This application form can be filled out and faxed into our office for processing or you can apply online through our secure connection.
- PCI Upgrade Application for Gas Stations - .pdf editable form
This credit application is only to be filled out by gas station owners and businesses operating in the retail petroleum industry. First Star Capital has a specific program available for the required PCI upgrade and this application is the starting point for that program.

Documentation checklist
This document serves as an outline for what information / documentation may be required to process a new financing request. Information on the checklist may vary depending on the dollar amount requested or the particular circumstances of the transaction request.
- Credit release authorization - .pdf editable form
This document may be needed to authorize First Star Capital to access bank, trade and borrowing references as well as credit bureau data.
- Business debt schedule - .pdf editable form
This document is typically only needed for refinancing requests. It details the current outstanding debt obligations and the underlying collateral allowing us to determine if it would be advantageous to refinance any existing loans, leases or notes.
- Facilities list - .pdf editable form
This document provides a detailed outline on a businesses’ current equipment that is owned. This is typically required on sale-lease back or refinance requests where the borrower is seeking working capital from equipment that they currently own.
- Personal financial statement - .pdf
Personal financial statement - Excel spreadsheet
This document is typically only required on commercial sized requests (upwards of $100K - $150K) and start up businesses. It lists the personal assets and liabilities of the owner(s) of the business.
The following 3 documents are only required for start up companies

Provides some background information on the owners, the concept of the new business and why it is expected to be successful.
- Sources and uses - .pdf editable form
Provides an outline of how funds will be allocated during the start up phase of a business. Also, allows us to understand how the business will be initially capitalized.
The following 3 documents are only to be completed by equipment vendors
- Vendor profile - .pdf editable form
This document is to be completed by new vendors that have not worked with First Star Capital in the past or for vendors that lack public record credit information that would allow us to verify good standing in their industry.
- Used vehicle condition report - .pdf editable form
This document is to be completed by the vendor prior to funding in a situation where the vehicle that First Star Capital is financing is used / pre-owned.
- Used equipment condition report - .pdf editable form
This document is to be completed by the vendor prior to funding in a situation where the equipment that First Star Capital is financing is used / pre-owned.
